Availability
This form is now on the ORU web site - use the link above and log in. You must list
each date separately. Please list travel restrictions for a particular date and
include if you are available Friday, Sunday, and/or weekdays. This way you don't
have to enter every week date that you are available.
Match Report
This document is in MS-Word format.
Save it to disk. Before you print out the file, set the printer to print in
landscape and select two-sided printing and the tablet option. Please
read the directions on the form. Clubs are to maintain copies of the form for
use. It is the home club's responsibility to turn the completed form (both home
and visitors roster) over to the referee before kick-off. Officials from both sides
(not the referee) are responsible for verifying CIPP enrollment before the match.
